Protection Plan Application
We are so glad you've made the decision to serve as Country Hills. As we serve minors under the age of 18 and vulnerable adults we need to follow our Protection Plan. In order to serve you will need to become a Protection Plan Approved Volunteer and you can follow the Application process below to do so.
Protection Plan Online & Refresher Training
As part of the approval process you will also need to complete training below before you begin serving. The following video will take you through all of the training and information you will need to successfully adhere to the Protection Plan standards at CHC. This same training needs to be completed annually as well.

Application Process
- Follow this link to fill out the 2019 Ministry Personnel Application Form For Family Ministries
- Read the 2024 Protection Plan
- Print and Sign the Confidentiality Covenant (return by email or in person at the interview)
- Print and Sign the Release of Information Form (return by email or in person at the interview)
- Send a copy of the Ministry Personnel Reference Fillable Form 2018 to each of your references. They can simply fill out the form digitally for you.
- Watch the Protection Plan Training Video
- Get a Police Check (instructions below)
To Get a Police Check
You will need to provide the church with an updated Police Records Check every five years. To get a Police Records Check, please follow these steps:
Waterloo Regional Police only do Police Checks online. Click on this link and follow the instructions on the website to start the application. You can give your receipt to the church office for reimbursement. You will receive your Record Check by email when it is complete. Please forward the email with all attachments to the church office office@countryhillschurch.ca when you receive it. When you are filling out the application, please make sure it is a Record Check with Vulnerable Sector Search. You may be asked to provide a Volunteer Agency Letter. If so, please contact the church office office@countryhillschurch.ca and we will provide one.
Approval Process
- Complete the Application Process
- Have an Interview with members of the Protection Plan Team (this will be done by phone or on Zoom)
- If everything is all clear the Protection Plan team will submit your name to the Board of Elders for Final Approval.
- Attend In-Person Training (when we offer if again) within 6 months of approval.
